 CEU Application

 APPLICATION FOR SOCIAL WORK CONTINUING   EDUCATION CREDIT

For Pre-Approval of Social Work Continuing Education Program Credits, your application and all the supporting documents must be completed and submitted to LCWTA at least two-weeks before the program begins.

Training facilitators are responsible for collecting attendance, administering the evaluation, maintaining a record of the evaluation results, and issuing the appropriate certificates as outlined in the application. 

Please provide attendance records and evaluation results or a summary of the evaluation results to LCWTA once the training is completed. Attendance records must be kept for three years following the event.

CEU Application Process

  1. Download the LCWTA Application for Social Work Continuing Education Credit.

  2. Complete the form and collect all required documents listed in the Attachment section on page 3 of the form.
